Ongoing discussions persist whether single lung transplantation (SLT) or bilateral lung transplantation (BLT) lead to better patient outcomes and quality of life. This study analyzed 58 peer-reviewed articles from 1990 to 2024, comparing SLT and BLT across several parameters, including indications for transplantation, overall survival, pulmonary hypertension, complications, chronic rejection, functional status, and quality of life outcomes. The review also addressed native lung complications, the future of lung transplantation, and the outcomes of non-transplanted lungs in SLT cases. Bilateral lung transplantation (BLT) may offer a more definitive solution for bilateral lung disease. However, the choice between single lung transplantation (SLT) and BLT should be personalized according to each patient's unique requirements. Further research is needed to elucidate each approach's specific advantages and limitations, enhance patient outcomes, and refine lung transplantation practices.
