We report a case of systemic capillary leak syndrome in a lung transplant recipient who developed recurrent episodes of volume overload, respiratory failure, and acute kidney injury in association with hypotension and hypoalbuminemia. Initiation of monthly high dose intravenous immunoglobulin prophylaxis effectively abrogated the syndrome.
